A Subject Access Request (SAR) is a request, under the Data Protection Act 2018, to an organisation asking for a copy of the personal data that it holds about you.

An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.

You can make a request for your health records here by creating an account and logging into the portal.

If you are an individual requesting records on behalf of a patient e.g. a family member or advocate, please complete the linked consent form and upload it to the relevant screen of the form. We will be unable to process requests without patient consent; if you believe consent is not required please state this within the form. Form Download

The fastest and most efficient way of making a subject access request, is online via our SAR Portal. Please create an account or login with your existing credentials.  With an account, you will be able to answer any questions we have concerning your request and track the progress we are making.  Please ensure you have 2 forms of ID ready before submitting your request and that you select the application form which matches the health records you are asking to receive.
