Summary
A Subject Access Request (SAR) is a request, under the Data Protection Act 2018, to an organisation asking for a copy of the personal data that it holds about you.
An SAR enables you to understand how and why an organisation is using your data and to check that they are doing it lawfully.
How to make a request

If you are an individual requesting records on behalf of a patient e.g. a family member or advocate, please complete the linked consent form and upload it to the relevant screen of the form.
